My wife knows I'd leave her if she got fat and vice versa. We think lack of caring for your body and appearance means you don't care about anything. Your significant other is probably the same way, if you can't committ to staying attractive and putting forth the effort, move on.

This shouldn't matter, because he's been with you for such a period of time that he should automatically see past your figure and etc. Sure, physical attraction is important in a relationship, but after a certain amount of time into the relationship it shouldn't be a big deal. After all, at the end of the day, we're all just shapes. When you're being intimate, obviously be as attractive as you can, but otherwise, be yourself. He doesn't like it, tell him to hit the road.
