1) I can't really comment on the beginning... I don't rememeber it all too well.
2) The director made everyone think that Bruce Willis was alive by showing him sitting with the mom and "talking" with her, The boy wasn't scared of him, the director made it look as though his wife was just distant and mad at him, rather than not seeing him at all.

at the end of the movie, Bruce Willis is already dead, which isnt shown the whole movie, it is hinted at a lot, because at the beginning he was killed, but you dont know for sure until the end.
The boy can see dead people, so he can see Bruce Willis, and he knows the entire movie that Bruce is dead, but he doesnt tell him. It's supposed to be like surprising and make you think like ooh i shoulda known aha
