It is more common to get just an MD or just a PhD -- though there are combined programs.
You do not enter after high school or two years of college. You generally have to get a bachelor's degree first -- which takes four years.
A stand-alone MD takes four years. It is usually followed by a one year internship and two to six years in a residency program.
A stand alone PhD takes four to eight years. There is usually about three years of coursework, followed by written and oral Comprehensive Exams, followed by a dissertation. The time it takes to write a dissertation is what adds the variability to the length of time it takes to get the PhD. There is no set length of time it takes to get a combined degree. I suspect that dong them together would take about six years at a minimum.
1. no you do not, you go to undergraduate school normally for 4 years where you have to finish certain classes.
2. after that you go to grad school for 4 years, if you go into med you have 3 more years of internships also

It would help to know which school's MD/PhD program you're thinking of, but in any case, these are graduate degrees.
After high school, you need to get a bachelor's degree first, meaning 4 years of undergraduate studies, during which you would take the pre-requisite sciences and other courses needed for the MD and PhD.
Medical school is another 4 years. If you want a PhD as well, it's another year or two, probably taken at the same time as the medical courses - but how it's done depends on the program and the school.
There is a joint BS/MD program offered by some schools that you can enter directly from high school. You'd be taking the med school courses at the same time as your bachelor's courses. These programs are small and extremely selective. Generally, students in them have taken AP courses and tests for most of the science and math pre-requisites.
One program you can do after 2 years in a bachelor's degree program is the Pharm.D., which is a doctorate in Pharmacy. It's the same level as aPhD, though it's not a PhD. It is NOT an MD.
