Transgender and Transsexual ?
They are the same.
A transexual is a type of transgendered. A transexual is a transgendered that is undergoing medical proceedures to change their gender presentation, like hormones and surgery
they're the same thing except transsexual means they've been physically altered to live as a member as their desired sex.
TRANSGENDER people are people who cross GENDER ROLES. They includes cross-dressers. In some societies, transgenderism also includes people who do work of the opposite gender, or are attracted to the same sex.
TRANSSEXUAL people, like myself, are facing or have faced some mismatch between GENDER IDENTITY/SUBCONSCIOUS SEX/BRAIN SEX and BODY SEX. It's mostly biological so transsexualism doesn't vary from society to society in the way that transgenderism does. Genderqueer, androgynous, and bigender people may blend or cross gender roles (making them transgender) and/or may feel a different subconscious sex from their body sex (making them transsexual).
Most transgender people are not transsexual, but many transsexual people are transgender. People use the term transgender to describe both groups and the concerns of each group. That said, transsexual people have many concerns, e.g. with medicine, documentation, etc. which may not matter to cissexual transgender people.

b. People who insist that subverting sex and gender is better than identifying with one sex or one gender. This includes some cis-LGB people as well as some trans people.
I'm a lesbian trans woman. I'm not straight, or femme, so the first group sometimes dismisses my identity. I'm not bisexual or bigender, so the second group sometimes dismisses my identity. I guess I'm one of the many stuck in between these two factions.

Although the superficial similarities (and the efforts of the transgender community) lead many people to believe the two are the same, they are NOT.
'Transgender' describes a person, male or female, who dresses, behaves or presents in a manner that differs from their gender norm. The majority of 'transgender' people are crossdressers/transvestites (including Drag Queens/Kings); it also includes social androgynes and 'genderqueers'. Despite the efforts of some transgender activists, it does NOT include transsexual people.
Transgender people do not need medical intervention, although some may choose to take hormones to aid their presentation.
'Transsexual' describes a person, male or female, who was born with a congenital neurological condition ( Benjamin's syndrome, or HBS; http://www.sexchange.us/hbs.html ) which means that their gender identity, which is determined in the hypothalamus of the brain, does not match their physical anatomy. Transsexual people require medical intervention, including hormone therapy and genital surgery, to correct this mismatch and lead normal lives, but transsexualism is neither defined by, nor restricted to, that intervention.
To be transgendered is feeling like you should be the opposite sex.
To be transsexual is having an operation that "makes" you the opposite sex.
They aren't two opposing ideas.
