Yes, implementing 911 cost them extra money, which means those costs have to be passed on to the consumer - you. The Telco's charge the VoIP services to access the 911 network. So, that increases cost and eats away at the competative edge of low-cost-calling.
One of the primary reasons that Skype does not support 911 is also because it would be a nighmare to implement and support. Keeping in mind that most Skype users use Skype from a laptop or while traveling. If you happen to be traveling to Florida and your Skype account is registered in NYC, then if you call 911 while in Florida where will they send the 911 response team?..... they will be looking for you in NY instead of in Florida... Implementing 911 is an problem issue for many VoIP services.
The Telco's again have a monopoly on access and eveyone else has to pay them fees to get access (if the Telco's will cooperate in the first place).
I believe there are currently initiatives in progress working on the 911 issue so that one day in the not too distant future, everyone will have access from anywhere and by any means. Obviously, we have a ways to go yet before we get there....
